The ideal candidate holds an active CNO RN/RPN license with a valid BLS certificate, strong communication skills, and experience in care coordination.

Qualifications

• Active CNO RN or RPN license is required.
• Valid BLS certificate is required.
• Excellent verbal and written communication skills; prior care coordination experience is an asset.

Responsabilités

• Conduct initial and ongoing in-home client assessments and contribute to care plans.
• Create and update care plans to match client needs and conditions.
• Explain care plans to staff and relay updates in a timely manner.
• Respond to client or family inquiries regarding care and condition.
• Communicate concerns or scheduling changes to clients and families.
• Monitor staff performance and attendance, and address issues as needed.
• Review shift notes and incident reports and take appropriate action.
