The role includes Saturday overnights and Sunday shifts, delivering client-centered nursing, administering medications, and ensuring safety and comfort in the home setting. Strong communication with family and Care Coordinators is essential.

Qualifikationen

• Current registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN) or RN.
• TB screening within the past 12 months is required.
• COVID-19 vaccination proof as per Ontario Human Rights Code exemptions.
• Vulnerable Sector Check within 12 months.
• Valid CPR/First Aid certification.
• WHMIS 2015 certification within the past 12 months.

About the Opportunity

The Care Company is looking for a compassionate, motivated, and dedicated Registered Practical Nurse (RPN) or Registered Nurse (RN) to provide one-on-one care for a 12-year-old female client in Toronto. We are looking for someone who is passionate about providing exceptional, client-centered care and building meaningful, long-term relationships with the individuals and families they support.

Schedule

We are seeking a nurse who can commit to a consistent, ongoing weekly schedule: Saturday Overnights: 9:00 PM - 7:00 AM Sundays: 2:00 PM - 7:00 PM The family is looking for a reliable and dedicated nurse who can become a consistent, long-term member of their care team. Please apply only if you are able to commit to regular, ongoing availability for these shifts.

Duties and Responsibilities

Provide compassionate, individualized nursing care in accordance with the client's care plan and within the scope of practice of a Registered Practical Nurse (RPN) or Registered Nurse (RN).

• Assist with activities of daily living (ADLs), including personal hygiene, bathing, repositioning, and other personal care needs.


• Administer medications and provide nursing interventions as prescribed and outlined in the client's care plan.


• Monitor and assess the client's physical and emotional well-being, recognizing and promptly reporting any changes in condition or concerns.


• Support the client's mobility, transfers, positioning, and prescribed exercises, as required.


• Ensure the client's safety and comfort, including the safe and appropriate use of medical equipment.


• Maintain accurate, timely, and thorough clinical documentation in accordance with organizational policies and professional nursing standards.


• Communicate effectively with the client's family, Care Coordinators, and other members of the healthcare team to ensure continuity of care.


• Practice safely and competently within your professional scope of practice, clinical knowledge, and demonstrated competencies.

Clinical Skills and Experience

Previous experience with the following clinical skills is preferred:

• Deep suctioning and respiratory assessment and monitoring.


• Oxygen therapy, including oxygen saturation monitoring and administration.


• Client repositioning, skin integrity assessment, and skin care.


• Pain assessment and management.


• Administration and monitoring of PRN (as-needed) medications.


• Training may be provided to the right candidate who demonstrates a willingness to learn and develop their clinical skills.

Additional Requirements

Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).

• Negative TB screening completed within the past 12 months.


• Proof of COVID-19 vaccination, subject to applicable exemptions under the Ontario Human Rights Code.


• Satisfactory Vulnerable Sector Check completed within the past 12 months.


• Valid CPR/First Aid certification.


• WHMIS 2015 certification completed within the past 12 months.


• Strong verbal and written communication skills in English.


• Ability to recognize, document, and promptly report changes in client condition or concerns regarding client care to the appropriate Care Coordinator.


• Ability to work independently while exercising sound clinical judgment and following established care plans and organizational policies.


• Access to a reliable vehicle and a valid driver\'s licence are considered assets but are not required.
