Lifemark Health Group through Cole Harbour Integrated Health Services – pt Health is seeking a Care Coordinator to join the Dartmouth area clinic team. The role supports a multidisciplinary team, ensures accurate patient intake, schedules appointments, handles payments and records, and maintains high standards of care in a collaborative setting.

Responsibilities

• Answer telephone calls and greet patients and visitors with excellent service.
• Coordinate intake information on first visits and throughout treatment plan.
• Assist patients with clinic paperwork to avoid errors.
• Schedule patient appointments and maintain medical records.
• Collect payment at time of service and track funding coverage.
• Data entry and billing responsibilities.
• Coordinate courier packages and process emails, mail, and faxes.
• Provide administrative support to the Care Coordinator Manager and Clinic Director.

Status: Permanent Full-Time

Location: Forest Hills Parkway, Dartmouth, NS
Expected Schedule: Monday – Friday

Primary Responsibilities

• Answer telephone calls, greet patients and visitors upon arrival, and provide excellent customer service for all questions or concerns
• Coordinate a detailed and accurate intake of information on the patient's first visit to the clinic and throughout their treatment plan
• Assist patients with completing paperwork at the clinic to avoid errors and inaccurate information
• Coordinateschedulingofpatient appointments and maintain the patient’smedical record
• Collect payment at time of service
• Track coverage for all funding streams to ensure approval
• Complete data entry and billing
• Coordinate courier packages (in or outgoing) and process incoming and outgoing email, mail and faxes
• Provide administrative support to the Care Coordinator Manager and Clinic Director

Qualifications & Core Competencies:

• High School Diploma, G.E.D. or equivalent
• High degree of organizational and multi-tasking skills in a busy and fast-paced work environment
• Preferably with 1-3 years work experience in a private clinic setting
• Excellent communication and interpersonal skills
• Computer savvy and data entry skills in a physiotherapy clinic setting will be an asset but not required
• Flexible and adaptable to change
• Ability to hit the ground running with minimum supervision
