Interior Health in Trail is seeking BCCSW registered Social Workers to join the Kootenay Boundary Regional Hospital on a casual basis. The role focuses on psychosocial assessments, crisis intervention, counselling and advocacy to support patients and families through illness, hospitalization and recovery.

You’ll work with interdisciplinary teams to develop patient-centred care plans, assist with discharge planning, connect patients to community resources, housing supports, financial assistance

Qualifications

• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid BC driver’s license.

Responsabilités

• Provide psychosocial assessments, crisis intervention, counselling and advocacy to help individuals navigate illness, hospitalization and recovery.
• Collaborate with physicians, nurses and allied health professionals to create patient-centred care plans addressing medical and social needs.
• Assist with discharge planning and connect patients to community resources, housing supports, financial assistance programs and home care services.
• Promote compassionate, culturally safe and client-centred practice to improve health outcomes for diverse populations in the Trail area.
