Jean Tweed Centre in Toronto is seeking a Case Manager (ELM) for full-time, on-site work. You will lead a wrap-around process to coordinate services, provide counselling, outreach, assessments, and referrals, and ensure timely case notes and data entry.
You will apply harm reduction, trauma-informed, women-centered and anti-oppressive practices while connecting clients to housing, health care and community resources in the Toronto area.

CA$28.84 - CA$33.73 per hour
150 Elizabeth St, Toronto, ON
9:00am-5:00pm (with a rotation of evenings and weekends)
