Job summary

The Victorian Order of Nurses (VON) is seeking a Case Coordinator to deliver comprehensive assessments and care planning for clients within a home and community setting. You will work with clients and caregivers to meet goals, manage budgets, and access needed resources.

This role emphasizes client self-management, adherence to VON policies, and collaboration with community partners to connect clients with suitable health and support services.

Qualifications

• University degree or college diploma in health or social services field.
• Minimum of 3 years related experience in home and community care or social services.
• Experience with validated assessment tools and community resource knowledge.

Responsibilities

• Interpret and complete standardized assessment tools (e.g., interRAI CHA).
• Develop a comprehensive care plan in collaboration with client and caregiver.
• Moster client progress and adjust care plans as required.
• Review expenditures and help clients manage within their budgets.
• Facilitate access to services and refer to community resources.
• Coordinate with partners to meet client needs and interventions.
