Job summary

Michael Garron Hospital in Toronto is seeking a Manager of Outreach and Integrations to lead teams involved in outreach and hospital-community integration. You will oversee staff across ET-PCNC, NLOT and INHCP, drive performance, and coordinate with community partners to improve patient transitions and flow.

You will shape initiatives that reduce non-acute LOS, reduce readmissions and support vulnerable patient populations through collaborative, process-driven leadership.

Qualifications

• Demonstrated experience in incremental leadership responsibilities in Hospital Operations.
• Familiarity with improvement process principles is a definite asset.
• Experience with community service, home care and/or primary care an asset.
• Demonstrates a process driven approach to problem solving.
• Excellent written and oral communication skills.
• Exceptional interpersonal skills.
• Demonstrated ability to multi-task and manage conflicting priorities.
• Ability to collaborate with internal and external partners.

Responsibilities

• Oversee teams: ET-PCNC, NLOT and INHCP, ensuring accountability and performance.
• Hire, support and manage staff and coordinate with external partners.
• Drive transitions from hospital to community settings and home care.
• Lead initiatives to reduce non-acute LOS, readmissions and ALC rates.
• Contribute to corporate quality improvement projects related to patient flow.
