Job summary

Ontario Health atHome is seeking a Hospital Care Coordinator to assess patient care needs and eligibility, link patients with community services, and coordinate care plans at Michael Garron Hospital in Toronto. Ideal candidates have regulatory memberships in Ontario and at least 1 year of community health experience, with strong communication and organizational skills, and a valid

Qualifications

  • Membership, in good standing, with Ontario regulatory bodies (e.g., CNO, CPTO, COT Ontario, Ontario College of SW & SSW, CODA, COASP).
  • 1+ years of recent experience in community health or related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
  • Strong time management and organizational abilities; able to work independently in a busy multidisciplinary environment.
  • Ability to accurately complete required documentation, reports and forms.
  • Valid driver’s license and access to a reliable vehicle.
  • Proficient in a Windows environment.
  • COVID-19 vaccination status as a condition of employment.

Responsibilities

  • Develop individualized care and service plans and link patients to service providers.
  • Coordinate and monitor delivery of care plans.

Ontario Health atHome is seeking qualified candidates to work at Michael Garron Hospital in a full-time permanent position.

Please note, while you may be initially assigned to Michael Garron Hospital as your primary location you may be required to be flexible and assist other locations as needed.

What will you do?
  • In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans

  • Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected
What must you have?
  • Membership, in good standing, with the applicable regulatory body:
    • College of Nurses of Ontario
    • College of Physiotherapists of Ontario
    • College of Occupational Therapists of Ontario
    • Ontario College of Social Workers and Social Service Workers
    • College of Dietitians of Ontario
    • College of Audiologists and Speech Language Pathologists of Ontario
  • 1+ years of recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision‑making skills
  • Effective time management, prioritization and organizational skills, with the ability to work independently and co‑operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports and forms
  • A valid driver’s license and access to a reliable vehicle
  • Proficient in a Windows environment
  • We have a mandatory COVID‑19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID‑19 vaccination status prior to start date.
What would give you the edge?
  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
