Job summary

NAANDWECHIGE-GAMIG Wikwemikong Health Centre seeks an Indigenous Transition Facilitator to coordinate care across clinical and non-clinical providers on the care continuum.

You will engage with clients, families and caregivers, attend hospital rounds, and support discharge planning and community reintegration with culturally appropriate services.

Qualifications

  • Registered RN/RPN in good standing with Ontario College of Nurses.
  • Social Worker in good standing with OCSWSSW.
  • Palliative care training for front line workers; LEAP asset.
  • Indigenous cultural competency training as organizational curriculum.
  • Minimum 2–5 years nursing or social work experience.
  • Indigenous lived experience.
  • Knowledge of Indigenous program funding, resources and legislation.
  • Ability to engage Indigenous communities/organizations respectfully.
  • Indigenous language ability is an asset.
  • Experience in Indigenous health/social services, hospital or community case management.
  • Proficiency with computers and EHR software.
  • Strong problem solving, time management and autonomous work.

Responsibilities

  • Be in contact with community members accessing medical services in hospital to assess needs.
  • Engage early with caregivers in care plan development and training needs.
  • Foster cross-disciplinary relationships to ensure smooth transitions across care sectors.
  • Utilize effective communication strategies; shared care plans and family meetings.
  • Meet performance indicators and collect data.

Job description

Indigenous Transition Facilitator
Summary

This resource will seek to integrate provincial, federal and other community health and social support services across the care continuum. It will improve access to quality care through a variety of providers, both clinical and non‑clinical. It will address the physical, social, emotional and spiritual care needs of the client.

Responsibilities
  • Attend medical rounds at local hospitals including Health Sciences North and Manitoulin Health Centre.
  • Be in contact with each community member accessing medical services in hospital to see if health/social care needs are required.
  • Ensure the early engagement of client identified caregivers in development of the care plan, and to determine any education or training they may require to support the client in their living arrangement of choice upon discharge from a regional facility
  • Relationship building across Trans disciplinary teams and community partners formalizing process to strengthen efficient transitions across the care sector; acute care to community and long‑term care.
  • Utilizing effective communication strategies; shared care plans, family meetings, and electronic health record benefits
  • Meet performance indicators and data collection
Qualifications
  • Registered Nurse/Registered Practical Nurse in good standing with the College of Nurses of Ontario OR Social Worker in good standing with the OCSWSSW, or other allied health professional with relevant skill set
  • Palliative Care for Front Line Workers in First Nations Communities training; Learning Essential Approaches to Palliative and End of Life Care (LEAP) an asset
  • Indigenous cultural competency training (organizational curriculum of choice)
  • Minimum of 2‑5 years nursing or social work experience
  • Indigenous lived experience.
  • Knowledge of system navigation, community resources, legislation, research and funding arrangements specific to Indigenous programs and services.
  • Ability to assist and support in processes of engaging and building mutually supportive relationships with local Indigenous communities/organizations in a community‑centered manner.
  • Awareness of formal patient or service agreements with local Indigenous communities/organizations.
  • Ability to speak an Indigenous language is an asset.
  • Experience in an Indigenous health and or social service organization, hospital or community‑based case management, health promotion, public health or social work is essential
  • Proficiency in the use of computers and various software applications including electronic health records applications
  • Demonstrated knowledge of health care system as it relates to Indigenous communities, or willingness to learn and engage with individual communities as required
  • Demonstrated problem solving and critical thinking
  • Demonstrated time management and ability to work autonomously
  • Excellence in communication, partnership building and inter professional care commitment
  • Demonstrated commitment to continuous learning and professional development
