Responsibilities

• Manage INTEGRAcare Clients including establishing and modifying dynamic care plans and schedules.
• Manage Care Teams by planning and scheduling Caregivers with focus on continuity and consistency of care for Clients.
• Help teach the new hires program and assist in the orientation process of new hires.
• Be part of the on-call rotation.
• Participate in company and team building events.

Reporting to Director of Nursing, the Care Manager is mainly responsible for providing primary care management, nursing support, coordinating & scheduling care for your Clients to ensure that they maintain their dignity, independence, and joyful living!

• Developing and maintaining dynamic care plans and schedules for our Clients
• Lead/coach teams of Caregivers and interact with our multidisciplinary team (Physiotherapists, massage therapists, doctors etc.)
• How to teach New Graduate PSWs various elements of caring for our Clients

As a Care Manager you will:

Type of Position: Full-Time

Annual Salary: $75,000-$77,000 + Benefits

Location: 396 Moore (on-site)
