As a
Care Coordinator (Status: Temporary, Full-Time, start date: June 2, 2025, to March 20, 2026; Location: Sudbury), you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.
Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.

What will you do?
- In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans
- Link patients with service providers
- Coordinate and monitor care plan delivery
- Establish a helping relationship with patients and their families
- Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected
What must you have?
- Registered Nurse, Physiotherapist, Occupational Therapist or Social Worker;
- Member in good standing with the College of Nurses of Ontario, the College of Physiotherapists of Ontario, the College of Occupational Therapists of Ontario or the Ontario College of Social Workers and Social Service workers;
- Relevant experience working in a health care environment;
- In-depth knowledge of home health care and other community resources in Ontario Health atHome;
- Demonstrated case management, assessment, and interviewing skills;
- Problem-solving, good judgement and decision-making skill and ability;
- Proven ability to prioritize professional duties, manage multiple patients, and efficiently organize workload;
- Established ability to accurately complete required documentation, reports, forms, and recommendations;
- Strong interpersonal and collaboration skills;
- Proven ability to communicate information effectively through a variety of means;
- Effective listening, observation, and facilitation skills;
- Good understanding of the roles of other health care professionals affiliated with Ontario Health atHome;
- Knowledge of funding agencies available to support patients;
- Practical knowledge and understanding of relevant legislation;
- Comprehensive knowledge of the standards of practice and professional guidelines set forth by the College of Nurses of Ontario;
- Working knowledge of computer software and Microsoft Office applications;
- Must have valid driver’s license and access to a vehicle;
- Advanced oral and written proficiency in English is essential.

What would give you the edge?
- Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
- Case management experience or recent related community experience
- Ability to speak French or another second language
