Job summary

Qualicare Home Care in Lethbridge is seeking a dedicated Care Coordinator to evaluate client needs and oversee the care delivered. You will develop care plans and ensure competency of caregivers, upholding Qualicare brand standards while supporting intake, assessments, health education, and follow-up care.

The role provides leadership across client programs, coordinates scheduling, and maintains accurate documentation in line with Alberta regulations.

Qualifications

  • Licensed Practical Nurse with current Alberta licensure.
  • Minimum five years of clinical experience, including home health.
  • Experience coordinating caregivers or interdisciplinary teams is an asset.
  • Strong clinical assessment, communication, and organization skills.
  • Familiarity with Alberta healthcare regulations and home care standards.

Responsibilities

  • Manage client referrals, intake, onboarding, and ongoing care coordination.

Qualicare Homecare is looking for compassionate, caring, and reliable individuals to join our family as Care Coordinators. The Care Coordinator plays an important role in coordinating client care, supporting caregivers, and ensuring high-quality service delivery.

Job Summary

The Care Coordinator is accountable and responsible for evaluating client needs, overseeing and monitoring the care delivered, developing an appropriate plan of care, and ensuring the competency of caregivers. The Care Coordinator commits to participate in establishing and implementing the highest standards of care.

The Care Coordinator provides leadership and direction in all aspects of client care. She/he functions and participates in the development, implementation, monitoring, and evaluation of client programs and services while upholding Qualicare brand standards. This individual will assist in the delivery of home care services including client intake and assessments, health education/counseling, and follow-up care.

Responsibilities
  • Manage client referrals, intake calls, service conversion, onboarding, and ongoing care coordination while ensuring compliance with Qualicare policies and regulations.
  • Conduct in-home assessments, identify client needs and goals, develop individualized care plans, and ensure the appropriate and safe level of care is provided.
  • Monitor service delivery, conduct regular client follow-ups, address concerns, and ensure care is provided according to client needs, care plans, and Qualicare standards.
  • Consult with physicians and healthcare professionals as required, coordinate skilled services under appropriate supervision, and maintain accurate client documentation.
  • Support client satisfaction, respond to concerns, and participate in community education and outreach activities to promote Qualicare services.
  • Schedule and match caregivers to new and existing client engagements, ensuring appropriate coverage and client-caregiver fit.
  • Support caregiver onboarding, orientation, in-service education, and continuing education, including reviewing client-specific care plans, safety expectations, and home routines.
  • Support HR in ensuring caregivers are familiar with applicable laws, regulations, and workplace requirements.
  • Respond to caregiver questions and concerns, investigate issues, and follow up promptly to support caregiver satisfaction.
  • Ensures high‑quality care by monitoring care plan adherence, reviewing care logs, and maintaining accurate documentation.
  • Identifies and reports concerns promptly while supporting continuous improvement in client care and service delivery.
  • Engage in ongoing professional development through targeted training, conferences, and goal‑setting to strengthen performance and support continuous growth.
  • Perform other duties as assigned
Qualifications
  • Licensed Practical Nurse with current unencumbered licensure to practice professional nursing in Alberta.
  • Minimum five (5) years of clinical experience with at least half of that time working in home health or related health care organization.
  • Experience supporting caregivers or interdisciplinary teams is an asset.
  • Strong clinical assessment, communication, organization, and problem -solving skills
  • Experience in care coordination, client assessments, scheduling, or healthcare administration is preferred.
  • Excellent customer service skills and a compassionate, client-focused approach
  • Ability to work independently, manage competing priorities, and make sound decisions
  • Comfortable using computers, scheduling software, and electronic documentation
  • Knowledge of Alberta healthcare regulations, home care standards, and provincial and federal compliance requirements; familiarity with ALA and CDHCI programs is an asset
