Job summary

Qualicare Toronto East & Downtown is seeking a compassionate RN to join our home care team as Care Coordinator. You will assess client needs, coordinate care plans, monitor services, and ensure caregiver competency while upholding Qualicare brand standards.

This role involves some direct patient care, health education, and collaboration with physicians and staff. It offers hybrid scheduling with both remote and on-site work, a competitive CAD 85,000 salary, and ongoing professional development

Qualifications

  • Registered Nurse or other regulated health professional designation with 2+ years of clinical experience.
  • Strong clinical skills.
  • Strong communication skills; ability to interpret clinical information and explain care plans.
  • Existing relationships in hospitals or community health networks.
  • Negative TB test or chest x-ray.
  • Certified in CPR.
  • Self-directing with the ability to work with little direct supervision.

Responsibilities

  • Assist business development efforts in the territory by accompanying the GM(s) to various events, speaking engagements and other community initiatives
  • Consults with supervisors and staff regarding clients, families and service programs
  • Conducts in-home assessments and creates care plans for clients
  • Consults with physicians and other care providers related to client care services
  • Performs supervisory visits to clients on a regular basis
  • Prepares, administers and appropriately documents actions taken specific to medications
  • Performs all skills, treatments, and procedures competently in accordance with the provincial scope of practice
  • Participates in employee selection process and supervises the overall performance of field teams
  • Provides backfill resources as needed for team

The Care Coordinator is accountable and responsible for evaluating client needs, overseeing and monitoring the care delivered, developing an appropriate plan of care, and ensuring the competency of caregivers. The Care Coordinator commits to participate in establishing and implementing the highest standards of care.

The Care Coordinator is responsible for the quality and adequacy of services provided and will provide leadership and direction in all aspects of client care. She/he functions and participates in the development, implementation, monitoring, and evaluation of client programs and services while upholding Qualicare brand standards. This leader will assist in the delivery of home care services including client assessments, health education/counseling, and also will provide some direct client care.

Qualifications

  • Existing relationships in hospitals or community health networks - business development is everyone’s responsibility!
  • Negative TB test or chest x-ray
  • Certified in CPR
  • Self-directing with the ability to work with little direct supervision

Compensation: $85,000.00 per year
