Job summary

Interior Health is seeking a Community Health Worker to join Invermere Home Support. You will assist with personal care, daily living activities, and home safety to promote client independence.

You’ll work with Nurses and LPNs to implement home health plans. Requirements include Grade 12, completion of a recognized Resident Care Aide/Home Support Worker program, registration with BC Care Aide and Community Health Worker, and a valid BC driver’s license with access to a vehicle.

Qualifications

  • Must have Grade 12 or equivalent.
  • Graduation from a recognized Resident Care Aide/Home Support Worker program.
  • Active BC Care Aide and Community Health Worker registry number required.

Responsibilities

  • Assist with activities of daily living for Home clients.
  • Provide personal care and support to promote independence at home.
  • Support home safety and daily living activities per care plan.
  • Observe changes in client condition and report to care team.
  • Collaborate with Nurses and LPNs in home health plans.
  • Document care provided and participate in team rounds.

About the Job

As a Community Health Worker, you will work as part of a dynamic team of Nurses and LPNs who support Community Clients' independence and ability to stay at home. Community Health Workers provide home support services to clients, by assisting with personal care and daily living, while promoting maximum independence. Community Health Workers are key players in the home health care plan for each client.

Typical duties include:

  • Activities of daily personal care, such as bathing, dressing, grooming and oral hygiene
  • Household duties related to maintaining a safe and healthy environment in the home
  • Meal preparation in accordance with designated plans
  • Assistance with mobility and transfers
  • Observing and reporting changes in client behaviour and condition
  • Performing delegated tasks according with training and procedures
Position Type

This is a Permanent Part Time (0.98) opportunity.

Shifts

Shifts of work will be scheduled days and nights rotating from 07:00 to 15:30, 08:00 to 16:30, 12:30 to 21:00 and 13:30 to 22:00 hours.

Qualfications
  • Grade 12
  • Graduation from a recognized Resident Care Aide/Home Support Worker Program, or an equivalent combination of education, training and relevant experience
  • BC Care Aide and Community Health Worker registry (please include your number)
  • Current valid BC Drivers' License and use of a reliable vehicle
