Job summary

Scarborough Health Network (SHN) is seeking a Social Worker (BSW) for the Supportive Housing Program. You will assess, treat and support residents with mental health needs to maintain housing and health, coordinating with care teams and community resources.

Responsibilities include intake assessments, care planning, crisis response, group programming, and helping residents navigate social services. Requires strong interpersonal skills, crisis intervention, and experience in community mental

Qualifications

• Bachelor of Social Work required.
• Current registration/certification with the College of Social Workers and Social Service Workers.
• Minimum 3 years experience in community mental health settings.
• Experience with residents in supportive housing or similar environments is preferred.
• Strong interpersonal and crisis intervention skills required.
• Ability to work collaboratively with a multidisciplinary team.

Responsibilities

• Assess and treat residents to support housing stability and health.
• Develop resident care plans and coordinate with care teams.
• Lead groups and therapeutic programming for community integration.
• Navigate social services and assist with system access for residents.
