Job summary

McKesson Canada is seeking bilingual Field Case Managers to join a growing Patient Support Program supporting patients with chronic and complex conditions. In this field-based role, you will serve as a trusted partner to patients, healthcare providers, clinics, and pharmacies throughout the treatment journey.

You will help patients gain timely access to therapy, navigate reimbursement processes, receive education and ongoing support, and stay engaged in their treatment plans.

Qualifications

• Bilingual in English and French, both written and spoken.
• Experience in a Patient Support Program (PSP).
• Postsecondary education or an equivalent blend of training and experience.
• Ability to manage multiple priorities in a dynamic setting.
• Strong organization, communication, and relationship-building skills.
• Proficient with MS Office and electronic documentation systems.

Responsibilities

• Serve as the primary point of contact for patients in the PSP.
• Assist patient enrollment and access activities for therapy.
• Educate patients on disease, treatment expectations, and adherence.
• Support patients throughout their treatment journey and identify barriers.
• Document patient assessments and ensure continuity of care.
• Collaborate with reimbursement specialists, clinics, and health professionals to facilitate access.
• Support access initiatives by collecting and analyzing information for reimbursement and coverage.
• Maintain patient follow-ups and escalate concerns when needed.
• Build and maintain strong relationships with physicians, nurses, clinics, pharmacists, and other health stakeholders.
• Provide program updates to health professionals while ensuring privacy compliance.
