Job summary

Serefin Health is seeking a Care Coordinator to support families during the transition to the Ontario Autism Program. The role focuses on determining needs, helping families understand service options, funding, and selecting qualified providers.

Responsibilities include conducting determinations, maintaining applicant data, and collaborating with teams to improve processes. Strong communication, empathy, and ability to work independently with flexible hours are essential.

Qualifications

  • Bachelor’s Degree with a minimum of 3 years of experience in human services, social services, or a clinically related discipline.
  • Family-centered, person-directed approach to support with focus on families.
  • Ability to navigate challenging conversations with compassion and empathy.
  • Experience working with families of children on the autism spectrum is an asset.
  • Proficiency with database management, email, and Microsoft Office.

Responsibilities

  • Educate families about the program and available services.
  • Maintain accurate applicant information in the database.
  • Assist families in making informed decisions regarding next steps for Core Clinical Services.
  • Empower applicants and communities to recognize and support Autistic Children and Youth.
  • Attend team meetings and training sessions to contribute to operational goals.
