Job summary

Champlain Rehab Solutions is seeking an Occupational Therapist to assess and provide consultation and interventions to clients in their homes, promoting independence through evidence-based practice. You will coordinate care with the client care team and problem-solve with clients to improve outcomes.

The role requires OT licensure in Ontario, a valid driver’s license, and the ability to work independently. Occasional weekend on-call and a supportive, growth-focused environment are offered.

Qualifications

• Degree in Occupational Therapy from a recognized school of Occupational Therapy.
• Member in good standing with the College of Occupational Therapists of Ontario.
• Valid Driver's License with access to a vehicle.
• Compassionate and empathetic.
• Self-driven with ability to work independently.
• Skilled in caseload management.
• Skilled at electronic documentation.
• Able to provide virtual care.
• A registered ADP authorizer (asset).

Responsibilities

• Assess and provide consultation, recommendations and interventions to clients in their homes.
• Coordinate delivery of care through communication with the client care team.
• Problem solve with client and care team to increase independence.
• Document assessment findings, treatment plans, progress and discharge summaries.
• Support safe client care through continuous quality improvement.
• Participate in weekend on-call roster within the geographical region.
