Job summary

Lifemark Meadowlark Centre in Edmonton, AB, is seeking an Active Rehabilitation Nurse (RN) to coordinate patient care within the rehabilitation program. The role blends clinical nursing with administration, care coordination, and quality reporting, acting as the communication hub among physicians, rehab staff, patients, and administration.

Qualifications

• Current RN registration in good standing with provincial body.
• Minimum 2 years nursing experience (rehabilitation/chronic pain preferred).
• Strong clinical assessment and critical thinking.
• Experience with electronic medical records and clinical documentation.
• Excellent organizational and time management skills.
• Strong interpersonal and written communication abilities.
• Ability to work independently while collaborating with a team.
• Proficiency with Word, Excel, and Outlook.

Responsibilities

• Coordinate and process physician referrals and orders for the rehab program.
• Manage patient intake, orientation, assessments, and internal referrals.
• Monitor patient flow and program capacity; maintain census reports.
• Maintain patient caselists and tracking from referral to discharge.
• Prepare and distribute weekly patient reports and scheduling info.
• Document patient progression, suspensions, and discharges.
• Review charts for clinical needs, follow up and care planning.
• Collaborate with physicians on care plans and clearances.
• Maintain accurate records and assist with CARF accreditation.
• Prepare weekly interdisciplinary meeting agendas and participate in meetings.
