Job summary

Lifemark Health Group in Edmonton is seeking an Active Rehabilitation Nurse (RN) to coordinate patient care within our multidisciplinary rehab program. The role blends nursing practice with care coordination, documentation, and program administration.

You will work with physicians and rehab teams to ensure timely referrals, accurate records, and smooth progression through the program, while maintaining a patient-centered approach.

Qualifications

• Current registration as a Registered Nurse (RN) in good standing with the applicable provincial regulatory body.
• Minimum 2 years of recent nursing experience (rehabilitation, chronic pain, musculoskeletal, community, or outpatient experience preferred).
• Strong clinical assessment and critical thinking skills.
• Experience with electronic medical records and clinical documentation.
• Excellent organizational and time management skills.
• Strong interpersonal and written communication abilities.
• Ability to work independently while collaborating within an interdisciplinary team.
• Proficiency with Microsoft Office, including Word, Excel, and Outlook.

Responsibilities

• Review and process physician referrals and rehabilitation orders.
• Coordinate patient intake, orientation, team assessments, and internal consult referrals.
• Monitor patient flow through the rehabilitation program to ensure appropriate scheduling and program capacity.
• Maintain patient caselists and ensure accurate tracking from referral through discharge.
• Prepare and maintain weekly patient census reports.
• Coordinate appointment requests and communicate scheduling information with patients and interdisciplinary team members.
• Monitor patient progression, suspensions, and discharges throughout the program.
• Maintain accurate and timely documentation in EMR & paper records as required.
• Assist with CARF accreditation documentation and reporting.
• Coordinate and prepare weekly interdisciplinary meeting agendas and participate in meetings.
