Job summary

Qualifications

• Bachelor of Pharmacy (BPharm) or Doctor of Pharmacy (PharmD) from an accredited institution.
• NHRA Pharmacist License or eligibility for licensure.
• Minimum 3 to 5 years of clinical pharmacy experience in a hospital or healthcare setting.
• Strong knowledge of pharmacotherapy, medication safety, and clinical pharmacy practices.

Responsibilities

• Review, assess and optimise medication therapy for safety and effectiveness.
• Participate in multidisciplinary rounds and provide evidence-based recommendations.
• Monitor patient response and adjust therapy to improve outcomes.

Job Summary

The Clinical Pharmacist is responsible for providing comprehensive clinical pharmacy services to ensure the safe, effective, and cost-efficient use of medications. The role collaborates closely with physicians, nurses, and other healthcare professionals to optimise medication therapy, improve patient outcomes, and support medication safety and quality initiatives in accordance with organisational policies and regulatory standards. The role is aligned with pharmaceutical care, medication management, and patient safety responsibilities identified within the M42 pharmacy framework.

Major Responsibilities

1. Review, assess, and optimise medication therapy by evaluating prescriptions for appropriateness, dosage, interactions, contraindications, and therapeutic effectiveness.
2. Participate in multidisciplinary clinical rounds and provide evidence-based medication recommendations to physicians and healthcare teams.
3. Monitor patient response to medication therapy and recommend adjustments to achieve optimal clinical outcomes.
4. Provide medication counselling and education to patients, families, and caregivers regarding medication use, side effects, and adherence.
5. Support medication safety programmes, including adverse drug reaction monitoring, medication error reporting, and risk mitigation initiatives.
6. Develop and implement clinical pharmacy protocols, guidelines, and formulary management activities based on current best practices and evidence-based medicine.
7. Collaborate with healthcare professionals to ensure compliance with medication management standards, accreditation requirements, and regulatory guidelines.
8. Conduct medication reconciliation during admission, transfer, and discharge processes to promote continuity of care.
9. Contribute to quality improvement projects, audits, and clinical research activities related to medication use and patient safety.
10. Provide education and training to healthcare staff, pharmacy interns, and students on medication-related topics and best practices.
11. Maintain accurate clinical documentation and pharmaceutical records in accordance with regulatory and organisational requirements.
12. Promote rational and cost-effective medication use while ensuring the highest standards of patient care and safety.
