Point Care Pty Ltd. is seeking dedicated Youth Workers to support children and young people in residential homes, emergency placements, supported accommodation and community-based programs across Queensland.

Responsibilities include assessing needs, developing and reviewing individualised care plans, monitoring progress and coordinating with families and service providers to ensure continuity of care. Mandatory training will be provided, including medication administration where required.

Formación

• NDIS Worker Screening Clearance, where applicable.
• Strong written and verbal communication skills.
• Ability to work collaboratively with young people, families, colleagues and external service providers.

Responsabilidades

• Assess the social, emotional, behavioural and developmental needs of children and young people.
• Contribute to development, implementation and review of individualised care and support plans.
• Monitor and evaluate progress against plan objectives; document outcomes and recommend adjustments.
• Respond to incidents and crisis situations in accordance with individual plans.
• Promote a safe, inclusive and supportive environment that respects rights and dignity.
• Liaise with families, case managers, schools and government agencies to coordinate support and promote continuity of care.
