Life Without Barriers seeks a Case Manager – Next Step Plus to support young people transitioning from out-of-home care in Toowoomba and surrounding Darling Downs. You will manage a caseload, deliver outreach-based, strengths-based case management, and help navigate housing, education, employment, health, wellbeing and independent living.

The role is a six-month maximum-term parental leave replacement with travel across the service area and collaboration with a supportive team.

Qualifications

• A relevant degree in Social Work, Psychology, Human Services, Social Science or a related discipline, or equivalent experience.
• Experience providing case management to young people with complex needs.
• Strong assessment, planning, engagement and advocacy skills.
• The ability to coordinate services and build effective stakeholder relationships.
• Excellent communication, documentation and organisational skills.
• An understanding of trauma-informed and strengths-based practice.
• The ability to work autonomously, manage competing priorities and maintain accurate case records.
• A current unrestricted Queensland Driver Licence.
