Job summary

Life Without Barriers is seeking a Case Manager (NSP) to support young people transitioning from out-of-home care in Toowoomba and surrounding areas. You will deliver outreach-based, strengths-based, family- and community-connected case management to help young people achieve housing, education, employment and independent living goals.

The role involves travel throughout the service area and working within a supportive team.

Qualifications

• Degree in Social Work, Psychology, Human Services or related discipline, or equivalent experience.
• Experience providing case management to young people with complex needs.
• Strong assessment, planning, engagement and advocacy skills.
• Ability to coordinate services and build effective stakeholder relationships.
• Current unrestricted Queensland Driver Licence.

Responsibilities

• Developing goal-focused case plans tailored to each young person's needs and aspirations.
• Supporting access to housing, education, employment, health and wellbeing services.
• Building independent living skills and strengthening family, cultural and community connections.
• Advocating for young people and coordinating services to achieve sustainable outcomes.
• Collaborating with Child Safety, community organisations and other stakeholders.
