UnitingSA is seeking a dedicated Tailored Learning Case Manager to support young people aged 12–25 at risk of disengagement. Based in Noarlunga Centre, you will engage with schools, community groups and referrals to help youth reconnect with education, training and employment.

You will assess needs, develop plans and advocate for positive outcomes, delivering programs that build resilience, life skills and pathways to opportunity. Join a values-driven organisation making a real difference.

Qualifications

• Experience providing strengths-based case management for young people.
• Ability to assess needs, develop individual plans and coordinate referrals.
• Experience facilitating youth-focused programs and activities.

Responsabilités

• Provide strengths-based case management to young people facing barriers to education, training and employment.
• Assess needs, develop individual plans, coordinate referrals and advocate for positive outcomes.
• Deliver engaging programs and activities that build resilience and life skills.
• Build and maintain relationships with schools, community organisations, and government stakeholders.
• Maintain records, reporting and compliance with funding requirements.
