Job summary

Swan Hill District Health is seeking a Youth Worker to support young people aged 12-25 through outreach, individual support, and community-based activities in Swan Hill, Victoria. The role focuses on helping youths set goals and access education, employment and wellbeing pathways.

You will deliver programs, engage with partners, and advocate for youth needs in a safe, inclusive setting. Some evenings, weekends and after-hours work may be required.

Qualifications

• Current satisfactory National Police Check results or willing to obtain.
• Working with Children’s Check, or willing to obtain
• Eligibility to work in Australia
• Evidence of immunisation history in accordance with the Health Services Act 1988 and mandatory healthcare worker vaccination requirements.

Responsibilities

• Engage and support young people aged 12-25 through outreach, individual support and community-based activities.
• Build trusting relationships with young people and assist them to identify goals and pathways that enhance their wellbeing, education, employment and community participation.
• Deliver youth-focused programs, events and engagement activities that promote connection, resilience and positive outcomes.
• Work collaboratively with the multidisciplinary headspace team and external stakeholders to ensure young people receive coordinated and responsive support.
• Advocate for the needs of young people and facilitate access to appropriate services, resources and opportunities.
• Contribute to a safe, inclusive and culturally responsive environment that encourages young people to participate and thrive.
• Some evening, weekend and after-hours work may be required.
