Job summary

Berry Street is seeking a Case Manager for the GOALS+ housing and support program based across Morwell and Traralgon. The role focuses on strengths-based, trauma-informed case management and practical support for young people aged 16–21 leaving care or experiencing homelessness.

The successful candidate will develop individualized plans, connect participants to housing, education and health services, and help build life skills to enable greater independence.

Qualifications

• Experience in youth case management, housing, homelessness or related services.
• Strong assessment, communication, advocacy and documentation skills.
• Diploma-level qualification in Social Work, Youth Work or a related discipline.

Responsibilities

• Provide strengths-based, trauma-informed case management and practical support to young people.
• Develop individual plans and connect participants with housing, education, employment, health and wellbeing supports.
• Help young people build practical life skills, manage risks and work toward stable housing and independence.
