Job summary

Berry Street Yooralla in Gippsland invites a dedicated Case Manager to join our Morwell team on a full-time, permanent basis. You will deliver intensive case management, advocacy and outreach to vulnerable young people aged 12–18, coordinating with families, carers and Child Protection.

You will work with local services to empower resilience, build positive connections and help youths reach safety and goals.

Qualifications

• Experience in outreach support and/or case management with young people.
• Capacity to understand and support young people, their families and carers.

Responsibilities

• Provide intensive case management, advocacy and outreach support to young people.
• Collaborate with families, carers, Child Protection and wider service networks.
• Empower youths to strengthen resilience and achieve goals.
