Job summary

Berry Street Yooralla is seeking a passionate Case Manager to join the South Eastern Case Management Services in Noble Park on Boonwurrung Country. This full‑time, permanent role provides intensive case management, advocacy and outreach support to vulnerable children and young people aged 12–18 who are involved with Child Protection and Youth Justice.

You will work with families, carers and wider services to empower young people toward safety and positive outcomes.

Qualifications

• Bachelor level qualification in Community Services, Education, Social Work or similar field is required for this role.
• Relevant experience working in case management.
• High energy and commitment to your clients, whilst balancing working within the parameters of the role.
• Experience undertaking contracted case management within DFFH, Child Protection or another similar agency will be looked on favourably.

Responsibilities

• Provide intensive case management, advocacy and outreach support to help young people achieve safety, stability and positive life outcomes.
• Work collaboratively with families, carers, Child Protection and wider community services to empower young people to strengthen their resilience, develop positive connections and achieve their goals.
