Job summary

Better Health Network is seeking an experienced Young Person's Support Worker to provide intensive case management, outreach and practical support to young people aged 15–25 across Port Phillip, including those in low-cost housing or at risk of homelessness.

Based at the Victorian Pride Centre in St Kilda, you will work with RhED as part of BHN’s Resilient Communities unit to promote health, safety and wellbeing while reducing stigma and barriers to care.

Qualifications

• Tertiary qualification in health, social work or related discipline.
• Experience with case management for marginalised young people.
• Trauma-informed, strengths-based and culturally safe approaches.
• Strong assessment, intervention and risk management skills.
• Crisis intervention experience.
• Knowledge of prevention, health promotion and harm-minimisation.
• Excellent written and verbal communication and time-management.

Responsibilities

• Provide intensive case management and individualised support to young people with complex needs.
• Undertake assertive outreach across community and supported accommodation settings.
• Complete client assessments, intervention planning, risk management and safety planning.
• Advocate for housing, health, counselling, legal, financial, education and employment services.
• Respond to clients experiencing crisis or requiring immediate support.
• Help clients develop practical living skills that support long-term housing stability.
• Build effective referral pathways and partnerships with community, health, housing and government services.
• Maintain accurate client records and contribute to program evaluation and quality improvement activities.

About The Role

Resourcing Health and Education in the Sex Industry (RhED) is a specialist program within BHN’s Resilient Communities unit. RhED provides health promotion, education, advocacy, and support to people working in the sex industry across Victoria. The program delivers a range of outreach and centre-based services designed to improve health, wellbeing, and safety, while reducing stigma and barriers to care.

As a Young Person's Support Worker within BHN's RhED team, you'll provide individualised, trauma-informed and strengths-based support across community, accommodation and service settings.

Your Responsibilities Will Include

• Providing intensive case management and individualised support to young people with complex needs
• Undertaking assertive outreach across community and supported accommodation settings
• Completing client assessments, intervention planning, risk management and safety planning
• Advocating for clients and supporting access to housing, health, counselling, legal, financial, education and employment services
• Responding to clients experiencing crisis or requiring immediate support
• Helping clients develop practical living skills that support long-term housing stability
• Building effective referral pathways and partnerships with community, health, housing and government services

Maintaining accurate client records and contributing to program evaluation and quality improvement activities

About You

You'll be an experienced community services professional who can build respectful relationships with young people, work confidently in complex situations and deliver inclusive, non-judgemental support.

To Be Successful, You'll Bring

• A tertiary qualification in health, social work or a related discipline, with professional registration where relevant
• Demonstrated case work or case management experience supporting marginalised young people with complex needs
• Experience applying trauma-informed, strengths-based and culturally safe approaches
• Well-developed assessment, intervention, risk management and safety planning skills
• Knowledge of crisis intervention and experience responding appropriately to crisis situations
• An understanding of prevention, health promotion and harm-minimisation principles
• Strong written and verbal communication, organisation and time-management skills
• The ability to work independently and collaboratively within a multidisciplinary team
• An unrestricted Victorian driver licence

Desirable

• Lived sex work experience
• Experience working with culturally diverse communities, including working with interpreters
• Knowledge of the Residential Tenancies Act 1997, Victorian homelessness services and housing options for young people
• Broad knowledge of community and government services and resources

This role is primarily based at Level 2, Victorian Pride Centre, 79-81 Fitzroy Street, St Kilda, with work across community settings and other BHN locations as required.
