Job summary

Sacred Heart Mission is seeking a dedicated practitioner to join Women’s House, an entry point for women and gender-diverse people experiencing homelessness. You will provide assessment, case planning, and case management within a feminist, trauma-informed framework, supporting participants toward safer housing and improved wellbeing.

Responsibilities include maintaining a caseload, coordinating with referring agencies, and fostering collaboration with local services.

Qualifications

• Experience with assessment and case planning and case management.
• Experience in homelessness, mental health or community settings.
• Feminist, intersectional and trauma-informed practice.
• Degree in Social Work, Psychology, OT or equivalent.

Your new role

Women’s House is an initial point of contact for women andgenderdiversepeople experiencing or at risk of homelessness. We strive towards offering a safeandsupportive space that offers brief intervention housing support and case management as well as showers, laundry, hot mealsandsocial inclusion. We interpret individual's experiences within a systematic framework that acknowledges external factors that perpetuate disadvantage; we commit to advocacyandaction towards a more equitableandjust society for the women andgenderdiversefolks we support.

Your day-to-day

• Utilising persistentandassertive engagement techniques to establishandmaintain a professional relationship with service participants
• Hold a caseload of 12 participants for up to 6 months at a time
• Coordinate case conferences, planningandexit planning in conjunction with referring agencies
• Developandfoster positive relationships with local servicesandagencies
• Monitor, progress,andachieve program targetsandoutcomes
• Supporting the engagement hub through regular rostered and adhoc shifts in the Women’s House

We're looking for someone:

• With experience using assessment and case planning and case management skills, who will be flexible and open to new and creative ways of achieving outcomes
• With experience working in homelessness, mental health and/or community settings
• Committed to working in a framework that is feminist, intersectional, and trauma informed
• With an appropriate degree relevant to the delivery of community services e.g. Social Work, Psychology, Occupational Therapy or equivalent.
