Job summary

Healthcare Australia (HCA) is expanding its Homecare My Way division in Melbourne CBD. The Care Manager will handle self-managed clients virtually, delivering excellent customer service and compliant reporting, without home visits.

The role collaborates with multiple teams to ensure high-quality services and great client experiences. You will guide care planning, promote services to drive growth, manage funding activities, and maintain accurate records while supporting onboarding and policy

Qualifications

• Understanding of Aged Care and health/community services.
• Experience in customer-focused environments.
• Excellent time management and prioritisation.
• Strong written and verbal communication.

Responsibilities

• Develop care plans that support quality outcomes for clients.
• Actively promote Homecare My Way services to meet growth targets.
• Complete funding and debt management activities.
• Ensure all client documentation and case notes are up to date and high quality.
• Support business development and onboarding with policy-aligned service delivery.
