AnglicareSA is seeking a dedicated Support Facilitator to work with young people who have lived care experience, supporting their transition to independent living through trauma-informed, strengths-based case management.

You will help navigate housing, tenancy options, finances, education and employment, build practical life skills, access services and resources, and advocate for young people’s voices in decision-making.

This full-time fixed-term contract is based at Kurralta Park.

Qualifications

• Bachelor’s Degree (AQF Level 7) in Social Work or demonstrated knowledge and competencies appropriate to Social Work.
• Experience working with Aboriginal and Torres Strait Islander young people, families, and communities effectively, using culturally responsive approaches.
• Experience in delivering trauma-informed practice.

Responsabilités

• Deliver trauma-informed, strengths-based case management and support coordination, including screening, risk assessment, goal setting and client-led planning.
• Support young people to develop independent living skills, navigate housing and tenancy options, build financial literacy and access education, training and employment opportunities.
• Coordinate referrals, advocate for young people and connect them with services, natural supports and community networks that strengthen wellbeing and independence.
• Facilitate collaborative, client-led decision-making and support young people to access grants and allowances that assist transitions from care.
• Build safe, culturally responsive relationships and work with families and service providers to keep young people’s needs central.
