Job summary

Community Living Options invites applications for a role in our Forensic and Specialist Transitions team. You will join a trauma-informed, psychologically aware group delivering evidence-based support to individuals with complex mental health needs, under the guidance of a Service Coordinator.

You will help people feel safe, supported, and empowered to grow and meet their goals, building routines, independence, and resilience within a values-driven environment.

Qualifications

• NDIS Worker Screening is required.
• Working with Children Check is required.
• A current Driver’s License and First Aid & CPR certificates are required.
• Certificate III in Individual Support (Disability/Aged Care) or similar qualification is preferred.

Responsibilities

• Build daily routines and life skills to support transitions into the community.
• Support emotional regulation and respond to behaviours with compassion and professionalism.
• Encourage independence through activities like cooking, community access, and public transport.
• Implement Behaviour Support Plans and contribute to risk assessments.
• Promote dignity, choice, and hope in all interactions.
