Job summary

DFP Recruitment seeks experienced professionals to deliver high-quality, person-centred support needs assessments for people living with disability. You will gather information, analyse evidence, and prepare clear reports to support decisions, using trauma-informed and culturally-safe approaches.

You will work flexibly across virtual and face-to-face settings, collaborating with teams, maintaining records, and upholding privacy and ethical practice.

Qualifications

• Experience conducting support needs assessments or similar person-centred assessments.

Responsibilities

• Conduct support needs assessments through guided conversations and information gathering.
• Analyse and synthesise participant information and external evidence to support evidence-based outcomes.
• Prepare clear, accurate and defensible written assessment reports.
• Apply policy, legislation and procedures using sound judgement and ethical practice.
• Communicate assessment processes and outcomes clearly across virtual and face-to-face settings.
• Escalate complex or high-risk matters appropriately and contribute to continuous service improvement.
• Collaborate across teams to support coordinated and inclusive participant outcomes.
• Maintain accurate records using digital systems while ensuring privacy and confidentiality.
