Job summary

Juniper is seeking a culturally responsive Care Partner to join the Home and Community Care team in Perth North Metro. You will manage government-funded programs, coordinate services, and lead Community Support Workers while delivering person-centred care with a trauma-aware approach.

Requirements include experience in aged care or community services, a minimum Certificate IV, a current driver’s licence, and National Police Clearance. A generous salary package and Juniperks discounts are offered.

Qualifications

• Experience in case management or coordination within aged care, disability, or community services.
• Strong understanding of aged care funding programs and person-centred care principles.
• Excellent communication and leadership skills.
• Ability to manage budgets, develop care plans, and support staff performance.
• Minimum Certificate IV in community services/aged care/health.
• Current driver’s licence and access to a reliable vehicle.
• National Police Clearance.

Responsibilities

• Manage and coordinate community-based aged care programs with a focus on customer goals and wellbeing.
• Lead and support Community Support Workers through recruitment, onboarding, supervision, and development.
• Monitor client health and wellbeing, ensuring early intervention and appropriate referrals.
• Deliver trauma-aware, healing-informed care that respects each individual’s story.
• Ensure compliance with legislation, funding guidelines, and Juniper’s policies.
• Promote a safe and healthy work environment through proactive WHS practices.
