Job summary

Juniper in Perth North Metro is seeking a culturally responsive Care Partner to join our Home and Community Care team. You will coordinate SAH and CHSP programs, build strong relationships with customers and families, and ensure care is delivered with respect.

You’ll lead Community Support Workers, monitor client health, and drive trauma-aware, healing-informed care while complying with the Aged Care Act 2024 and Juniper policies. This role demands compassion, teamwork and a hands-on approach.

Qualifications

• Experience in case management or coordination within aged care, disability, or community services.
• Strong understanding of aged care funding programs and person-centred care.
• Excellent communication and leadership skills.
• Ability to manage budgets, develop care plans, and support staff performance.
• Current driver’s licence and access to a reliable vehicle; National Police Clearance.

Responsibilities

• Manage and coordinate community-based aged care programs with a focus on customer goals and wellbeing
• Lead and support Community Support Workers through recruitment, onboarding, supervision, and development
• Monitor client health and wellbeing, ensuring early intervention and appropriate referrals
• Deliver trauma-aware, healing-informed care that respects each individual’s story
• Ensure compliance with legislation, funding guidelines, and Juniper’s policies
• Promote a safe and healthy work environment through proactive WHS practices
