Job summary

Juniper is seeking a culturally responsive Care Partner to join the Home and Community Care team in Perth North Metro. You will manage government-funded programs, coordinate services, and lead Community Support Workers while delivering person-centred care with a trauma-aware approach.

About the Role

Juniper is seeking a culturally responsive and community-focused Care Partner to join our Home and Community Care (HCC) team in Perth North Metro. In this vital role, you'll be the trusted connection for our customers and their families, building strong relationships, coordinating services, and ensuring care is delivered with respect.

You'll oversee a portfolio of government-funded programs including Support at Home (SAH) and Commonwealth Home Support Program (CHSP). Your leadership will ensure services meet the highest standards of care and compliance under the Aged Care Act 2024 and the Strengthened Aged Care Quality Standards.
