Job summary

Juniper Aged Care is seeking a culturally responsive Care Partner to join the Home and Community Care team in Perth North Metro. You will coordinate government-funded programs and act as the trusted connection for clients and families, delivering respectful, person-centred care.

In this role you will lead a team of Community Support Workers, monitor wellbeing, manage services and ensure compliance with the Aged Care Act 2024 and quality standards.

Qualifications

• Experience coordinating aged care or community services.
• Knowledge of funded programs SAH and CHSP.
• Strong communication and leadership skills.
• Budgeting and care plan development experience.
• Current driver’s licence and vehicle.
• National Police Clearance.

Responsibilities

• Coordinate community-based aged care programs.
• Lead Community Support Workers through recruitment and onboarding.
• Monitor client health and facilitate referrals.
• Provide trauma-informed, person-centred care.
• Ensure compliance with legislation, funding guidelines, and Juniper policies.
• Promote WHS practices to maintain a safe workplace.
