Job summary

Kanda is seeking a dedicated TCP Coordinator to guide clients from hospital discharge through home-based care, coordinating plans and collaborating with hospital teams, funding bodies and field staff. The role emphasizes clinical coordination and patient safety.

You will manage assessments, care plans, referrals and reporting to ensure smooth transitions and reliable home recovery for clients in the Underdale area. AHPRA registration as an Enrolled Nurse is required.

Qualifications

• Current unrestricted AHPRA registration as an Enrolled Nurse.
• Experience in case management, care coordination or similar clinical role.
• Understanding of aged care, My Aged Care, referral pathways and community supports.

Responsibilities

• Conduct comprehensive assessments and establish or review TCP care plans per guidelines.
• Coordinate total care by liaising with hospital TCP coordinators and funding bodies.
• Arrange services, monitor client progress and delegate tasks with Senior Clinical Nurse.
• Provide clear care plans for field staff and participate in hospital TCP case management meetings.
• Manage referrals, occupancy, invoicing and weekly reporting for the hospital TCP team.
• Identify and report risks, incidents and complaints and escalate unresolved issues.
