Yorke and Northern Local Health Network in Port Broughton is seeking an Enrolled Nurse to coordinate the Transition Care Program (TCP). You will assess client suitability, coordinate admissions, and set individual goals to support independence and safe return home.

Working with patients, families and the care team, you will coordinate allied health services during admission and prepare for discharge, under the Nursing Unit Manager.

Qualifications

• Enrolled Nurse with NMBA registration and current practising certificate.

Responsibilities

• Undertake suitability assessments and coordinate admissions to the health service.
• Establish goals and plans that promote independence and a return to home.
• Support home visits and family conferences.
• Coordinate allied health services during admission and preparation for returning home.
