Job summary

Palliative Care SA is seeking an Enrolled Nurse - TCP Coordinator for Port Broughton. This permanent part-time role (32 hours per week) supports the Transition Care Program across Yorke and Northern Local Health Network, coordinating admissions, setting goals and developing discharge plans to promote independence at home.

You will undertake suitability assessments, work with patients to establish goals, support home visits and coordinate allied health services during admission and on return

Qualifications

• Enrolled Nurse with eligibility for enrolment and current practising certificate.
• Permanent Part Time Contract (32 hours per week).
• Location: Port Broughton, Yorke and Northern Local Health Network.

Responsibilities

• Undertake suitability assessments and coordinate admissions to the health service.
• Establish goals and plans with patients to promote independence.
• Support home visits and family conferences.
• Coordinate allied health services during admission and preparation for returning home.
