Job summary
South Western Sydney Local Health District is seeking an Enrolled Nurse for the Transitional Aged Care Program to join the residential-based team. The role focuses on reablement of older adults transitioning from hospital to the community, requiring assessment, goal setting and discharge planning with a supportive MDT.

Qualifications
- Current registration as Enrolled Nurse (Division 2) with AHPRA.
- Experience with older adults in aged care, rehabilitation, community, or transitional care.
- Ability to work independently within a multidisciplinary team.
- Strong clinical judgement and person-centred care.
- Knowledge of community nursing assessments and discharge planning.
Responsibilities
- Assess TACP clients in residential aged care facilities.
- Develop client-centered intervention plans.
- Set holistic goals and discharge planning.
- Collaborate with a multidisciplinary team including a registered nurse.
