Job summary

Hertfordshire County Council in Apsley is seeking an experienced Team Manager within the Integrated Discharge Service to lead a busy multi-disciplinary team delivering discharge to assess pathways and promote home independence.

You will oversee safeguarding, personalisation, and budgets, travel across hospital sites including Luton, Stevenage and Stoke Mandeville, and ensure high-quality, person-centred care with strong performance management.

Qualifications

• Experience in safeguarding practice and working with people with dementia and long-term conditions.
• Strong understanding of personalisation and Individual Budgets.
• Proven leadership and service improvement in a busy, high-demand environment.

Responsibilities

• Lead day-to-day practice within the team and ensure compliance with statutory duties.
• Manage team performance and discharge to assess pathways and reviews.
• Inspire and develop a team of Social Workers and Community Care Officers.
• Build strong partnerships with hospital and community services.
• Oversee budget management and quality improvement activities.
• Act as lead manager for safeguarding enquiries.
