St John of God Midland Private Hospital is seeking a Discharge Coordinator on a fixed-term basis from 30 November 2026 to 1 August 2027. You will deliver patient-centred discharge planning across the care team, aiming for safe, timely transitions from hospital to home and other settings.

The role requires RN/Midwife registration with NMBA, leadership and quality improvement experience, and strong communication skills.

Qualifikationen

• Registration with the Nursing and Midwifery Board of Australia as a Registered Nurse/Midwife.
• Experience in specialty leadership and quality improvement.
• Strong interpersonal and communication skills.
