Job summary

Home in Place is seeking a compassionate Tailored Support Coordinator to join our Community Participation team on the Central Coast. The role focuses on assessments, planning and tailored support to occupants in social and affordable housing, helping them access services to improve wellbeing and independence.

You will work with tenants, community organisations and internal teams to promote social inclusion, education, employment and long-term positive life outcomes.

Qualifications

• Relevant tertiary qualifications in Community Services, Human Services, Health or related discipline.
• WWCC current and valid is required.
• Minimum three years' experience in Community Services or related field.
• Experience in assessments, case planning and support coordination.
• Strong communication, relationship-building and stakeholder engagement skills.

Responsibilities

• Conduct strengths-based assessments and develop individualised support plans with occupants.
• Coordinate referrals to internal and external support services and monitor outcomes.
• Support occupants experiencing personal crisis, complex needs or tenancy risk.
• Identify opportunities that enhance wellbeing, education, employment and independent living outcomes.
• Build strong partnerships with community organisations and service providers.
• Work collaboratively with tenancy teams to improve outcomes for occupants.
• Facilitate tenant engagement initiatives and community participation programs.
• Maintain accurate case notes, assessments, reporting and compliance documentation.
• Contribute to service improvement initiatives and innovative support practices.

About the Role

Based at our Central Coast office, this role provides assessment, planning and tailored support to occupants living in social and affordable housing communities. Working closely with tenants, community organisations and internal teams, you will help people access the services and resources they need to improve their wellbeing, maintain successful tenancies and work towards greater independence.

You’ll play a key role in supporting occupants experiencing personal challenges or tenancy risk, while also identifying opportunities that promote social inclusion, education, employment and community participation.

Key Responsibilities

You Will Bring

• Relevant tertiary qualifications in Community Services, Human Services, Health or a related discipline.
• Current and valid Working With Children Check (WWCC).
• Minimum three years' experience in Community Services or a related field.
• Demonstrated experience conducting assessments, case planning and support coordination.
• Strong communication, relationship-building and stakeholder engagement skills.
• A person-centred and strengths-based approach to service delivery.
• The ability to manage competing priorities and work with a high level of autonomy.
• Excellent problem-solving skills and sound professional judgement.
• Strong IT skills, including experience using Microsoft Office applications and client management systems.
