Job summary

UnitingCare Queensland seeks a Supported Living Facilitator to assist clients living in the community, supporting independence and daily living in their own homes.

You'll travel between visits, delivering in-home personal care, companionship and activities per each Care Plan; CPR/First Aid is required; own vehicle and license are essential. Casual hours are mainly Mon–Fri 9am–5pm.

Qualifications

• Must have clear spoken and written English.
• Experience in Disability, Aged or Community sector is ideal but not essential.
• Current CPR and First Aid Certification is required before starting.
• Open Drivers License and access to a vehicle is essential.
• Car Insurance in your name is required.

Responsibilities

• Provide in-home personal care and daily living assistance.
• Drive between clients with your own vehicle.
• Hours are primarily Monday to Friday, 9am–5pm.
• Follow individualised Care Plans and support community activities.
• Assist with transportation, shopping and appointments.

• Employment Term: Casual
• Remuneration: $ 45.28 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team

You’ll provide care to your clients living in the community, supporting their independence and improving your quality of life. With no two days ever the same, you’ll spend your time on the road completing your scheduled visits, interacting one-on-one with clients and their loved ones within their homes completing activities that include:

• In home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing
• Must be available to drive between clients with your own vehicle
• Hours are mainly Monday to Friday between the hours of 9am-5pm
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments

• Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers
• Able to effectively communicate in spoken and written English
• Experience working in the Disability, Aged and Community sector is ideal but not essential – as long as you have positive attitude and a willingness to learn
• Current CPR and First Aid Certificate (required before you start)
• Open Drivers License and access to vehicle
• Car Insurance that is in your name
