Job summary

UnitingCare Queensland's FaDS team is seeking a Supported Living Facilitator to help clients live independently in their homes. You will provide in-home care, assist with daily tasks, and support engagement in community activities across Toowoomba and surrounding areas.

The role offers casual hours with a focus on flexible scheduling, the chance to grow within a Not-for-Profit organisation, and access to NFP salary packaging to maximise take-home pay.

Qualifications

  • Must have current NDIS Worker Screening Check.
  • CPR and First Aid Certification required.
  • Open Driver's Licence and access to a vehicle.
  • Excellent spoken and written English.
  • Disability/aged/community sector experience preferred but not essential.

Responsibilities

  • Provide in-home personal care and daily living assistance.
  • Drive between clients using your own vehicle.
  • Support independence and improve quality of life for clients.
  • Assist with community-based activities, appointments and shopping.
  • Follow individual Care Plans and coordinate with the FaDS team.

Supported Living Facilitator| Disability Services, QCSS Employment Term: Casual Location: Toowoomba Remuneration: $45.28 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team

You’ll provide care to your clients living in the community, supporting their independence and improving their quality of life.

  • In home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing
  • Must be available to drive between clients with your own vehicle
  • Hours are mainly Monday to Friday between the hours of 9am-5pm
  • Providing care in accordance with their individualised Care Plan
  • Supporting with community based recreational and social activities
  • Transportation and assistance with shopping and appointments
What makes a difference for us?
  • Able to work effectively with others in a team‑oriented environment and provide excellent support to clients and co‑workers
  • Able to effectively communicate in spoken and written English
  • Experience working in the Disability, Aged and Community sector is ideal but not essential – as long as you have a positive attitude and a willingness to learn
  • A current NDIS Worker screening check
  • Current CPR and First Aid Certificate (required before you start)
  • Open Drivers Licence and access to vehicle
  • Car Insurance that is in your name
