Claro Disability Services – Gladesville NSW is seeking a compassionate Support Worker to assist clients with complex needs in the community. You’ll tailor supports to individual goals, work with families and health professionals, and build meaningful relationships through flexible shifts and client-centered planning.

Role focuses on mobility and daily tasks, medical liaison with the care team, and promoting autonomy in a caring, respectful environment.

Qualifications

Responsibilities

• Person-centered planning: Chat with the client (and family/advocates), get to know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices as outlined in the plan.
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
