Job summary

Claro Aged Care And Disability Services in Belmont is seeking a compassionate Support Worker to assist clients at home and in the community. You’ll help with mobility, daily activities, medical supports, and meaningful social participation, while respecting clients’ goals and culture.

Responsibilities

• Person-centered planning: chat with client and family to understand goals and routines.
• Complex physical care: assist with mobility, transfers, positioning, daily tasks, and devices per plan.
• Medical supports: monitor changes, follow clinical plans, liaise with care team when needed.
• Social time and participation: provide meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: support decision-making, respect consent, promote independence where possible.

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
